Is your child in Year 10?

Provide your consent for their routine school vaccinations online.

In Year 10, students are offered the meningococcal ACWY vaccine.  This will take place in our school library on Monday 26th February, 2024.

To provide online consent you will need:

  • The link provided by our school.  Click here for the link  
  • Your Service NSW log-in details
  • Medicare card details for you and your child

If you or your child do not have a Medicare card, consent can still be provided by requesting a paper consent form directly from our school administration office.
